Visitor Access Form

All visitors to Metal Trades must have their visit approved by a designated MTI representative prior to entering the facility. To ensure the visit request is approved in a timely manner, all visit requests and required forms should be completed and submitted 24 hours prior to visit. Admittance will not be granted without prior notification/approval. In order to request access to Metal Trades facility, the following steps must be taken.

  1. The individual must complete the online Visitor Access Form. Please ensure all applicable fields are completed. NOTE: Incomplete forms will not be processed.
  2. Prior to granting access, it will be verified that the individual has completed all the required paperwork and is listed on a currently approved visitor list.
  3. Once access is approved, the individual be contacted via email that their visit is authorized, and access granted.
